Hi! I’m Jennifer.

I started Solid Earth Nursing Pathways because I’ve seen how overwhelming healthcare can be, especially when you’re managing multiple conditions, doctors, and medications on your own.

As a nurse, I’ve worked closely with patients who were doing their best to keep up with complex care, often without clear guidance or support. It’s a lot to carry, and too often, people are left to figure it out alone.

This work is about changing that.

I help people feel more steady, informed, and supported as they navigate their health whether that means making sense of a doctor’s visit, staying organized, or simply having someone in their corner.

I have four years of experience in hospital care and two years in case management, working primarily with patients who have higher medical needs and complex care situations.

I earned my Bachelor of Science in Nursing and am currently pursuing a Master of Public Health, with a goal of helping improve how patients and families experience and access care on a larger scale.

Outside of work, I’m a mom of two boys as well as two dogs (Tucker and Poppy) and two cats (Ridley and Hamilton). I live in Franklin and love the slower pace and small-town feel.

Services

I provide personalized support to help you navigate your health with clarity and confidence. Whether you’re managing a chronic condition, recovering after a hospital stay, or feeling overwhelmed by multiple providers and medications, services are tailored to meet you where you are.

All services are private pay, as I am not currently able to bill insurance. However, I strive to keep care accessible—pricing may be adjusted based on individual financial needs. HSA/FSA payments are accepted, and reduced rates are available for Medicaid patients.

    • 1 monthly visit which including doctor office coordination, education, health assessment, and medication reviews

    • One phone call 2x per month with RN

    • Ongoing messaging support between patient and nurse (1-2 business day response time)

    • Education on health conditions and medication review

    • $100/month

    • 2–3 check-ins per month (phone or in-person)

    • Coordination with healthcare providers as needed

    • Health assessments and medication review

    • Ongoing messaging support (1–2 business day response time)

    • Coaching and education for managing complex health conditions

    • Appointment preparation OR follow-up review

    • $300/month

    • 1–2 check-ins per week (not always guaranteed 2)

    • 1 RN-accompanied appointment per month

    • Coordination with providers (as needed)

    • Medication management (including review + guidance; hands-on setup as needed)

    • Appointment preparation AND follow-up

    • Ongoing messaging support (1–2 business day response time)

    • Coaching on complex conditions and basic dietary guidance

    • 25% off additional services

    • $400/month

  • Either one time service, add on service, or a build your own monthly service plan.

    • Hospital Discharge / Transition Support — $100

    • Medication Setup (every other week) — $75/month

    • Medication Management (setup + refill coordination) — $100/month

    • Health Assessment & Coaching Session — $100

    • Advanced Directive Planning & Notarization — $100

    • Additional Visit — $75/hour

    • RN-Escorted Doctor Appointment — $150

    • Dietary Coaching & Meal Planning — $100
